Sitagliptin Phosphate

FDA Approval: 16 October 2006
Indication: Adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus.
Pharmacology: A DPP-4 inhibitor which slows the inactivation of incretin hormones (e.g., glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP)).  It decreases glucagon levels and increases insulin release.
Usual Dose: 100 mg orally once daily.
Contraindications: None known
Warnings: Dosage adjustment is recommended in patients with moderate renal insufficiency (CrCl 30 to <50 mL/min), patients with severe renal insufficiency (CrCl <30 mL/min), or with ESRD requiring hemodialysis or peritoneal dialysis.

It should not be used in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis.

Pregnancy Category: B
Side Effects: Monotherapy:  nasopharyngitis

Combination with Pioglitazone:  Upper respiratory tract infection  and headache.

Drug Interactions: Monitor patients on digoxin.
